THE ACCESSORY PORT HAS BEEN RECEIVED FOR FAILURE ANALYSIS EVALUATION. A VISUAL INSPECTION WAS PERFORMED AND THE ACCESS PORT WAS FOUND TO HAVE BROKEN TAB ON THE CHAMBER RETAINER SNAP COLLAR PORT. THE BROKEN PIECE WAS RETURNED WITH THE ACCESS PORT KIT.
